Looks like a Seagate 20917-300 EC. Should be ST-02 family of cards, pretty common.

Its 50 pin scsi and floppy w/ bios. What do you mean by pinouts? Its just SCSI and Floppy...Do you need the jumper settings?

Pin-out for what? Both floppy and SCSI pin-outs are standard and well-documented.
