You could have insufficient voltage/current when writing the EEPROM/EPROMs, or your programmer is just flaky.
I'd suggest trying a different programmer. I've had good luck with the GQ 4x4 with an external 9v power supply. They're a bit pricey with the EEPROM expansion board, but well worth it.
