Reddit came through for me! Turns out the game was "Celtic Tales: Balor of the Evil Eye". It looks like its from 1995, which explains how I missed it in my other searches. Funny, this means it must have been relatively new when it arrived at my house.

the funny part is, the 1 reply the guy freely admitted he was just guessing, but his guess happened to be right. A lot of my descriptions were way off, but as soon as I saw the cover and read the name I recognized it instantly. Even turns out I've had the manual this whole time.
