Key Disk Copy-Protection

Just out of curiosity, how did Key Disk Copy-Protection work? It's not that I'm trying to crack the copyright protection on something - I'm just curious as to how it works.
 
From what I recall, the name refers to a form of copy protection that requires a certain floppy disk (the 'key disk') be mounted in order to run the program from a hard drive. A similar method requires a hardware 'key' (dongle) to be installed, usually into the serial connector.
