I can also recommend the Applesauce FDC, which can write Amiga floppies, along with many, many other formats. It can also flux image Amiga floppies.

New Methods for making Amiga floppy disks

I've been using ADTWin successfully. It's not ideal, but I had everything needed to make it work on hand.
It's very picky about which PC I use, but once I found a motherboard that worked, it has been reliable. I'm using an early P4 running XP. It doesn't verify the data written, so it's a good idea to check your floppies for errors before use (format them in a PC and reject ones with bad sectors).

I got you covered. All you need is a serial null-modem cable and/or a usb-serial adapter, as you only need to connect RX<>TX and GND, 3 wires.
