The strange case of XT-IDE that only works with a CGA card...

Try programming the EEPROM in the XT-IDE on the system you intend to use it in, using XTIDECFG from the ZIP file I linked. If that fails, then we know there's an issue with either your computer or the ROM circuit. The ROM circuit is extremely simple and easy to debug.
