Model II/16 DOM+CP/M

Several years ago I came across a DOM card for the Model II/16 series that included Lifeboat CP/M. I used it in my Model 16 for awhile before pulling it out & putting it on the shelf. Fast-forward to now, I've pulled it out to use it in a Model II, the boot floppy is no longer readable, and I haven't the foggiest idea where I purchased it (ebay?) or who the vendor was.

Does anyone recognize this card and know the vendor? Or know if there is an image of this disk archived somewhere? or have a copy & would be willing to image it?

Failing that, does anyone know what BIOS changes are necessary to use the DOM disk with CP/M on the Model II ?

Apparently, Lifeboat Associates still exist as Wayside Technology Group. It might be worthwhile to try to contact them, in case they might have something:

The vendor of that card would have been Ian Mavric in Australia. As he wrote in his product write-up in the June 2018 TRS8BIT, "Due to the age of the 8in media I can locate and supply, please make many backups of the boot disk as soon as you get your DOM Carte". But I'm sure he'll be happy to email you an image. Find his email address at the top of his website.
 
I also produced a Lifeboat CP/M that booted from floppy and ran off a DOM. Have a search on the forum for details. The software is called XDRIVER and it is still downloadable. I did ask Ian to host it on his Model II downloads page but nothing came of the request.

The hardware comprises a board called HANS-01 (Model II card adapter) and the LoTech IDE board which was originally designed for the Model IV. Again, searching the forum for these keywords should yield more information.

I also posted some YouTube videos showing how it was connected and demonstrating the software.
